A new patient must complete and return the new patient packet (download above) before they can be scheduled. This way we ensure when you start with us we have an overview of your active problems and history. The ways to return the new patient packet are to email to stultztelemedicine@gmail.com

Patients must provide a copy of insurance card(s), photo ID, and finished new patient packet prior to scheduling.

Co-pays and deductibles as dictated by your insurance plan or self-pays are due at the time service is provided. We accept major credit/debit cards, checks, money orders, and cash payments for these services.
